Application Procedures

To apply, the following information must be submitted:
  • a complete application
  • curriculum vitae
  • three letters of recommendation
  • a statement of the applicant's long-term career goal and field of interest
  • notarized copies of applicant's transcripts for USMLE Steps 1, 2, and 3
  • copies of ECFMG scores for Steps I, II, and III (if applicable)
  • An unmounted recent photograph of applicant must accompany application or be provided at time of interview.

You may submit an online application,(NOTE: to complete the application procedure, be sure to mail or FAX the additional information listed above) OR you may download an application that can be mailed or faxed. NOTE: To complete the application procedure, be sure to mail this entire application along with the additional information listed above to:
